This is a list of the differences between Ted Culkin's Super Mario comic story arc: The Millennial Door, and its first game adaptation Paper Mario: the Thousand Year Door (Gamecube, 2004). Its second game adaptation Mario Inc.: The Millennial Door (3DS, 2012) follows the story much closer and therefore does not have many plot- changing differences. 

Between the comic and the first game
  • Characters present in original book, but not game:
    • Prince Cream, the younger brother of Princess Peach, is present, studying in Rougeport. Peach comes to Rougeport so she can visit him. When Peach goes missing, her tries to find Peach, and save her himself, but he is captured by the Shadow Sirens right before Mario and crew enter to save both Peach and him.
      • Cream was studying with Goombella, and she therefore knows him and is trying to find where he is when he is not home one day.
    • The comic begins with Merlon reciting to Peach a prophecy about how a demon and its followers would each die in a gruesome way. Except for one who redeems and one who was never truly evil to begin with. This is referring to Crump, Sir Gordus, and the Shadow Sirens.
      • In fact, the motives for their actions derive from their hearing of the prophecy.
    • Iggy joins Bowser in his quest to find out who kidnapped Peach this time. He typically serves as a Greek Chorus to what his uncle is doing. But when it comes time for Bedlam to kill Cream for extended life, he pulls a gambit on them similar to what Jack Sparrow does in On Stranger's Tides.
    • Peach gives Mario and Co. her power to stop the Shadow Queen. But tells them to go and save Cream from the extended life ritual before exorcising her. Though Mario and Co. fail to do so, Iggy does while they finish the Shadow Queen.
      • Cream is saved, and also gains a few additional years to his natural life thanks to Bedlam. That said, when Peach comes to check on him after the exorcism, Cream is not emotionally stable.
      • Cream later hugs Iggy near the end as thanks for saving his life.
    • Princess Daisy is featured in the first part of Bowser's sub-story. Where he finds that Peach is absent, and Daisy has been told to take over for several days.
  • Differences to remaining characters.
    • Luigi's adventure is greatly expanded upon in the form of various short gags and sub-stories. Eventually, Luigi is slapped by Princess Daisy, who thinks he cheated on her.
    • Vivian says in a moment that she was once male, unlike her sisters, who were always female. However, there was one day a failed hexing ritual which changed Vivan's gender permanently. This was referenced in non- English translations of the game. Where Bedlam abuses Vivan by calling her a "boy".
    • Grodus and Bedlam desire to use a life extension ritual akin to that of the Fountain of youth seen in On Stranger's Tides. The idea is that water from the Rougeport sewers, along with water from a fountain behind the Millenial door, will give the years of the victim to those who drink of the Sewer and Millennial Door water mixed. At the cost of the life of the victim who drink only of the water from behind the Millennial Door.
    • Grodus and Bedlam also are in a seemingly romantic relationship together. And plan to each take a victim for the life extension ritual that I just mentioned. Though Bedlam's planned victim is never known, and she uses Cream instead when Grodus is killed.
    • Various partners are seen interacting with each other. Typically with Bomberry acting as a mentor to Koops. Or Goombella and Vivan acting like sisters to each other. These are obviously not seen in TTYD or the second game due to time.
    • Bowser is left disturbed by the events he witnessed with the Shadow Queen, and is present at the end near the Rougeport docks looking absolutely disturbed.
    • The aforementioned prophecy from Merlon is fulfilled when the following villains go through these fates.
      • Vivan was never truly evil, and immediately turns to Mario's side when she gets the chance.
      • Marylyn repents in a scene where she stops Lord Crump responding to Iggy's interference by trying to kill Cream. She then lives a happy life with Vivian after the game.
      • Sir Grodus is killed by the Shadow Queen when she zaps him. However, X-Nauts technology is able to restore him as a cyborg, but leading to a repentant character.
      • Bedlam tries to put Cream through the life extension ritual, but Iggy intervenes and tricks them into drinking the wrong potions. Which means that Bedlam dies and Cream gets an extended life span.
      • Lord Crump is furious when he sees the last superior of his killed. So he tries to slit Cream's throat in response. But Marylyn pins his feet down. While Peach, having been finally exorcized, sneaks up behind and breaks his neck, killing him. 
    • Differences in settings.
      • The train from Chapter 6 is portrayed more like a realistic 50s era train. While this is not the case with the more fantasy like version from the game.
Since this is all fake and not a real series of differences, I think I tried. Of course, in my dream world, Mr. Ted Culkin would be able to incorporate everything into the original comic story.
